The Food and Beverage License is an annual license related to the regulation of food sanitation for all food service establishments, and retail food stores operating in the City Of Rock Island. All new establishments, as well as those which undergo extensive remodeling, must submit a Food Plan Review Application along with a $50.00 application fee. Prior to approval of an application for a license, the City Health Inspector shall inspect the food service establishment or retail food store to determine compliance with the requirements of the Illinois Food Service Sanitation Code. Mobile Food Units and Seasonal Concession Stands, must also submit an annual Food and Beverage Renewal Application. A separate Temporary Food and Beverage Application must be submitted for all Temporary Food Stands.
All applications must be submitted to the Inspection Department, located on second floor in City Hall, 1528 Third Avenue, Rock Island.

Organizations registered in Illinois as not for profit and churches and schools serving food no more than once each week and not open to the general public, will be exempt from an annual fee.
